Specimen monthly CaaS report

Illustrative specimen or blank worksheet. Not a signed contract, legal opinion, client case or evidence that Dyasol has performed these operations. Complete and agree the relevant terms before use.

Decision summary

FICTIONAL period: August 2026, Example Supplier Ltd. Agreed capacity: 8 hours; narrow scope: recurring customer-evidence review. Recommendation for the next period: reduce or pause this narrow work if no new review or material change is due, subject to agreement. This is a proposal, not an automatic contractual pause or reduction right. No client engagement or measured Dyasol outcome is represented.

Work completed and time

2h reviewed the fictional 12-entry evidence index; 2h reconciled questionnaire statements with fictional control-owner notes; 1h working meeting; 2h updated actions and prepared management note; 1h quality review. Total: 8h. Three fictional entries were outdated: E04 access review, E07 recovery evidence, E11 subprocessor list. E04 and E11 were replaced with approved current references; E07 remains unverified. A reference update does not demonstrate a technical restore test.

Results and limitations

R01: approved response library now identifies a scope, owner and review date for all 12 fictional entries. R02: two stale references replaced; preserved prior customer submission unchanged. R03: no restore test performed; claim that recovery meets business needs is not approved. No technical implementation, incident response or full regulatory assurance was included. These are illustrative accounting and reporting entries only.

Dependencies and actions

A01 โ€” client IT owner: obtain approved recovery-test scope and environment by the next planning meeting. A02 โ€” commercial owner: ask the customer whether a partial answer plus plan is acceptable. A03 โ€” management: approve or decline a separately scoped restore exercise. Report unresolved risk honestly; an approval to investigate is not risk remediation.

Capacity recommendation

Reduce: if only quarterly evidence review remains, propose a lower recurring effort at an agreed commercial arrangement. Pause: if there is no useful agreed work and dependencies prevent progress, discuss a pause under written terms rather than consuming hours without a result. Hire or increase internal capacity: if frequent operational decisions, line management or required availability exceed part-time support, recommend an internal role or another delivery arrangement. No option creates a right to change existing payment or notice terms unilaterally.

Management decisions and next review

Decision requested: select ongoing cadence, approve A03 if justified, name the owner of E07. Record: decision [ ], authority [ ], date [ ], rationale [ ], next review [ ]. Attach the versioned evidence index, time record and action log. Acceptance checks: arithmetic reconciles; completed work has references; unverified claims are visible; next actions are executable.
